This full-time on-site role requires PA RN licensure, 1 year MDS, LTC and Care Plan experience; compensation is $40–$43 per hour with benefits including health insurance, 403(b), and paid holidays.

Responsibilities

Registered Nurse Assessment Coordinator- MDS Coordinator – RNAC

We are seeking to hire aRegistered Nurse Assessment Coordinator – RNAC to assist with accurate and timely completion of both the RAI and Care Management process from admission to discharge.

• $40 to $43 per hour based on experience.
• Full-time Monday through Friday.
• Fully On-site.

Qualifications:

• Licensed as a Registered Nurse - RN in Pennsylvania.
• Minimum of 1 year of MDS experience.
• Minimum of 1 year of Long-Term Care experience.

Job Summary:

• Complete all required sections of the MDS.
• Intensive focus on Care Plans: creation, documentation, monitoring and leading the Care Plan meetings.
• Ensure documentation is present in the medical record to support MDS coding.
• Work in collaboration with the IDT to assess the needs and formulate a plan of care for the residents.
• Obtain and ensure Skilled Certification/Re-Certification documentation is accurate.
