Job summary

St. Anne Home in Greensburg, PA seeks a Registered Nurse Assessment Coordinator – MDS Coordinator (RNAC) for full-time, on-site work.

Licensed RN in Pennsylvania with MDS and care plan experience preferred, to manage MDS submissions and care coordination from admission to discharge. The RNAC will partner with the IDT to develop and monitor care plans, ensure proper documentation for MDS coding, and maintain accurate certification records.

Qualifications

• MDS experience required.
• Care plan experience required.
• Experience in Long-Term Care setting preferred.

Responsibilities

• Complete all required sections of the MDS.
• Lead and document Care Plan meetings.
• Ensure documentation supports MDS coding in medical records.
• Collaborate with the Interdisciplinary Team (IDT) to assess needs and plan care.
• Obtain and verify Skilled Certification/Re-Certification documentation.

Registered Nurse Assessment Coordinator- MDS Coordinator – RNAC

We are seeking to hire aRegistered Nurse Assessment Coordinator – RNAC to assist with accurate and timely completion of both the RAI and Care Management process from admission to discharge.

• $40 to $43 per hour based on experience.
• Full-time Monday through Friday.
• Fully On-site.

Qualifications:

• Licensed as a Registered Nurse - RN in Pennsylvania.
• Minimum of 1 year of MDS experience.
• Minimum of 1 year of Long-Term Care experience.
• Minimum of 1 year of Care Plan experience.

Job Summary:

• Complete all required sections of the MDS.
• Intensive focus on Care Plans: creation, documentation, monitoring and leading the Care Plan meetings.
• Ensure documentation is present in the medical record to support MDS coding.
• Work in collaboration with the IDT to assess the needs and formulate a plan of care for the residents.
• Obtain and ensure Skilled Certification/Re-Certification documentation is accurate.
