Holy Family Senior Living in Bethlehem, PA is seeking a compassionate RN MDS Coordinator (RNAC) to join our team. This full-time day-shift role focuses on accurate MDS/RAI completion, care plan development, and coordination with the interdisciplinary team to ensure optimal resident outcomes.

Qualified applicants are RN-licensed in Pennsylvania, with a 2-year degree preferred and RAC certification preferred. Prior long-term care experience and knowledge of PPS/PDPM are highly valued.

Qualifications

• Licensed RN in Pennsylvania.
• 2-year college degree preferred.
• RAC certification preferred.
• At least 1 year of long-term care experience.
• Knowledge of MDS and care plan processes.
• Understanding PPS, State Case Mix and PDPM.

Responsibilities

• Complete and coordinate the RAI/MDS assessment process for assigned residents, ensuring accurate documentation and timely submissions.
• Develop comprehensive resident care plans at admission, annually, and after significant changes to optimize outcomes.
• Coordinate with the IDT, schedule assessments, and lead MDS-related meetings as needed.
• Monitor clinical documentation for accuracy, regulatory compliance, and proper coding, including 7-day look-back.
• Support reimbursement and case-mix determination through correct MDS coding and PDPM data.
• Maintain up-to-date knowledge of MDS/RAI requirements and prepare information for insurers.
