Job summary

St. Anne Home in Greensburg, PA, is hiring a Registered Nurse Assessment Coordinator (RNAC) for an on-site, full-time role, Monday–Friday. Pay is $40–$43 per hour, based on experience.

You must hold an active PA RN license and have at least 1 year of MDS, Long-Term Care, and Care Plan experience. You will partner with the IDT to assess needs, create and monitor Care Plans, and ensure accurate documentation and certifications.

Qualifications

• Licensed RN in Pennsylvania with active license.
• Minimum 1 year of MDS experience.
• Minimum 1 year of Long-Term Care experience.
• Minimum 1 year of Care Plan experience.

Responsibilities

• Complete all required sections of the MDS.
• Intensive focus on Care Plans: creation, documentation, monitoring and leading the Care Plan meetings.
• Ensure documentation is present in the medical record to support MDS coding.
• Work with the IDT to assess resident needs and formulate a plan of care.
• Obtain and ensure Skilled Certification/Re-Certification documentation is accurate.
