The role requires reliability, strong organization, and collaboration across disciplines to support high-quality resident care. You will oversee admissions, annual and quarterly assessments, and change-of-condition reporting while maintaining precise documentation and data entry in MDS software.

Qualifications

• Experience in geriatric nursing and resident-centered care.
• Strong assessment and documentation skills.
• Proficient communication and collaboration with interdisciplinary teams.

Responsibilities

• Coordinate resident assessments (admission, annual, quarterly, Medicare, change-of-condition).
• Ensure regulatory compliance for MDS processes.
• Review MDS and care plan files for accuracy and organization.
• Maintain data entry and reporting in MDS software.
• Lead interdisciplinary meetings for care planning and coordination.
• Update care plans based on assessments and resident status.
• Generate Quality Indicators Reports for leadership.
• Submit MDS data per reporting standards.
• Stay current with industry standards and best practices.
• Coordinate ancillary data reporting to billing.
