Job summary

Nexion Health, Inc. is seeking a MDS Coordinator in Flower Mound, Texas. The role requires nursing licensure (RN/LPN) and Graduate of an accredited nursing program to coordinate and complete MDS assessments with accuracy and timeliness.

You will collaborate with the interdisciplinary team on care planning, ensure regulatory compliance, and support accurate reimbursement. The position offers a comprehensive benefits package and professional development opportunities.

Understanding of MDS process and resident assessments.

Responsibilities

Coordinates and completes comprehensive resident assessments (MDS) with accuracy and timeliness.
Serves as a resource for resident care planning and conducts orientation sessions on the MDS process.
Collaborates with the interdisciplinary team on care planning and supports regulatory compliance and reimbursement accuracy.
