Job summary

Xenia Health and Rehab is seeking an Assistant Director of Nursing/MDS Coordinator for our skilled nursing and rehabilitation center at 126 Wilson Drive in Xenia, Ohio. This hybrid role combines MDS oversight with DON support.

You will own the MDS process, coordinate assessments, and help ensure regulatory compliance while guiding the nursing team.

The position requires an active Ohio RN license and prior MDS and SNF experience, with a commitment to compassionate, outcome-oriented care.

Qualifications

• Active Ohio RN license.
• Minimum Data Set (MDS) coordination experience.
• Skilled nursing facility / long-term care experience.

Responsibilities

• Complete and coordinate accurate MDS assessments and care plans under PDPM.
• Manage the MDS schedule, ARDs, and transmissions.
• Support the DON with day-to-day clinical operations and nursing quality.
• Lead survey readiness and regulatory compliance.
• Serve as a clinical resource and step in for the DON when needed.
