Xenia Health and Rehab is seeking an Assistant Director of Nursing/MDS Coordinator for our skilled nursing and rehabilitation center at 126 Wilson Drive in Xenia, Ohio. This hybrid role combines MDS oversight with DON support.

Xenia Health and Rehab is hiring an Assistant Director of Nursing/MDS Coordinator for our skilled nursing and rehabilitation center at 126 Wilson Drive in Xenia, Ohio. This Assistant Director of Nursing/MDS Coordinator role is a hybrid seat: you'll own the Minimum Data Set (MDS) process for the building and support the Director of Nursing (DON) as Assistant Director of Nursing (ADON). Prior MDS experience and prior skilled nursing facility (SNF) / long-term care experience are both required.

• Serve as a clinical resource and step in for the DON when needed
