Job summary

Apploi in the Dayton, Ohio area is seeking an experienced MDS Coordinator to join its leadership team. You will oversee the MDS assessment process, work closely with the DON and inter disciplinary team to ensure accurate, timely, and reimbursement-ready documentation.

Known for a warm, close-knit culture where staff and residents know each other by name, this SNF/LTC setting requires prior MDS experience, PDPM knowledge, and strong regulatory compliance.

Qualifications

• Active Ohio RN or LPN license.
• MDS experience required in SNF/LTC.
• Experience coordinating MDS assessments and care plans.
• Ability to lead interdisciplinary care-plan meetings.

Responsibilities

• Complete and submit MDS assessments and care plans accurately and on schedule.
• Coordinate with nursing, therapy, and social services to capture resident status.
• Monitor PDPM accuracy and Quality Measures.
• Ensure compliance with state and federal MDS regulations.
• Lead interdisciplinary care-plan meetings.
