Southview Acres Healthcare Center in St. Paul, MN is seeking an experienced MDS Coordinator (RN) to join our clinical leadership team. You will manage the RAI process, ensure accurate documentation, and support high-quality resident care.

The role requires MN RN license, 1–2 years MDS/RAI experience, and knowledge of PDPM and ICD-10. You will collaborate with a interdisciplinary team, participate in care plan meetings, and stay current with CMS regulations.

Qualifications

• Must hold an active Minnesota RN license.
• Minimum 1–2 years MDS experience preferred.
• Strong knowledge of RAI process, ICD-10 coding, and PDPM.

Responsibilities

• Coordinate and complete MDS assessments (including OBRA, PPS, and any applicable state-specific assessments).
• Ensure accuracy and timeliness of all MDS documentation and submissions.
• Collaborate with interdisciplinary team members to develop and update comprehensive care plans.
• Monitor case mix index (CMI) and identify opportunities for optimization.
• Ensure compliance with CMS guidelines and Minnesota state regulations.
• Participate in care plan meetings and communicate effectively with residents and families.
• Assist with audits, surveys, and quality improvement initiatives.
• Stay up to date on regulatory changes and reimbursement updates.

Job Title: MDS Coordinator (RN) Location: Southview Acres Healthcare Center – St. Paul, MN Employment Type: Full-Time

Position Summary

Southview Acres Healthcare Center is seeking a detail-oriented and experienced MDS Coordinator to join our clinical leadership team. The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process in accordance with federal and state regulations, ensuring accurate reimbursement, and supporting high-quality resident care.

Key Responsibilities

• Coordinate and complete MDS assessments (including OBRA, PPS, and any applicable state-specific assessments)


• Ensure accuracy and timeliness of all MDS documentation and submissions


• Collaborate with interdisciplinary team members to develop and update comprehensive care plans


• Monitor case mix index (CMI) and identify opportunities for optimization


• Ensure compliance with CMS guidelines and Minnesota state regulations


• Participate in care plan meetings and communicate effectively with residents and families


• Assist with audits, surveys, and quality improvement initiatives


• Stay up to date on regulatory changes and reimbursement updates

Qualifications
