We are seeking an experienced, detail-oriented, and clinical-minded MDS Coordinator (FT) to manage the Minimum Data Set (MDS) process and Resident Assessment Instrument (RAI) function for our skilled nursing and rehabilitation center.
In this role, you will lead the coordination, completion, and submission of accurate MDS 3.0 assessments in accordance with state and federal regulations, PDPM reimbursement guidelines, and facility standards. You will work closely with our interdisciplinary care team to ensure that each resident receives personalized, comprehensive, and compliant care plans.
MDS 3.0 & RAI Management: Oversee the scheduling, completion, and electronic transmission of MDS assessments, CAA reports, and care plans within required CMS/State timelines.
Interdisciplinary Collaboration: Lead weekly MDS/Care Plan meetings with nursing, therapy, dietary, social services, and activities teams to deliver holistic care planning.
PDPM & Reimbursement Alignment: Ensure clinical documentation accurately reflects resident care needs, acuity levels, and services rendered to support appropriate reimbursement under PDPM and Medicare/Medicaid programs.
Compliance & Quality Improvement: Monitor regulatory changes, conduct clinical audits, and participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
Care Planning: Create, revise, and maintain comprehensive, individualized resident care plans based on completed assessments and clinical changes.
Education & Support: Serve as a subject matter expert for interdisciplinary staff regarding documentation requirements and MDS updates.
Licensure: Active, unencumbered Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of South Carolina (RN preferred).
Experience: Minimum of 1–2 years of hands-on experience as an MDS Coordinator in a skilled nursing facility (SNF) or long-term care setting.
Technical Expertise: Thorough knowledge of CMS MDS 3.0 requirements, RAI guidelines, PDPM, and Medicare/Medicaid regulations.
Certifications: RAC-CT (Resident Assessment Coordinator-Certified) through AAPACN is highly desirable (or willingness to obtain).
Skills: Exceptional organization, clinical assessment skills, attention to detail, and strong communication skills. Experience with electronic health record (EHR) software (e.g., PointClickCare) is preferred.
