Job summary

Montcare Potomac is seeking an experienced MDS Coordinator (RN) in Maryland to oversee the RAI process, ensure timely MDS assessments, and support care planning in a skilled nursing setting.

The role requires a Maryland RN license with at least 2 years of MDS experience and strong knowledge of CMS guidelines. Proficiency with EMR systems and collaboration with interdisciplinary teams are essential.

Qualifications

  • Active Registered Nurse license in Maryland.
  • Minimum of 2 years of MDS experience in skilled nursing or long-term care.
  • Strong knowledge of the RAI process and CMS guidelines.
  • Experience with electronic medical records (EMR) systems.
  • Excellent assessment, organizational and communication skills.
  • Ability to work independently and collaboratively.

Responsibilities

  • Coordinate and complete MDS assessments per CMS guidelines.
  • Ensure timely and accurate submission of MDS assessments.
  • Develop, implement and update individualized care plans with the interdisciplinary team.
  • Monitor documentation for regulatory compliance.
  • Participate in Medicare and Medicaid reimbursement processes.
  • Conduct care plan meetings with residents and families.
  • Educate and support nursing staff on MDS processes and documentation.
  • Perform audits and quality assurance reviews related to MDS and care planning.
  • Stay current with regulatory changes and best practices.

Salary: $80,000 – $90,000 annually (based on experience)

Position Summary

We are seeking an experienced and detail-oriented MDS Coordinator (Registered Nurse) to join our team in Maryland. The MDS Coordinator is responsible for overseeing the Resident Assessment Instrument (RAI) process, ensuring accurate and timely completion of MDS assessments, care planning, and compliance with state and federal regulations.

Key Responsibilities
  • Coordinate and complete Minimum Data Set (MDS) assessments in accordance with CMS guidelines
  • Ensure timely and accurate submission of MDS assessments
  • Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team
  • Monitor documentation to ensure compliance with regulatory standards
  • Participate in Medicare and Medicaid reimbursement processes
  • Conduct care plan meetings with residents and families
  • Educate and support nursing staff on MDS processes and documentation requirements
  • Perform audits and quality assurance reviews related to MDS and care planning
  • Stay current with changes in regulations and best practices
