Northgate PostAcute Care is seeking an experienced MDS-RN (MDS Coordinator) to manage the RAI/MDS process, care planning, and regulatory compliance for residents in our California facility.

The role requires an active California RN license and at least 1 year of MDS/NDS coordination in a skilled nursing setting, with strong knowledge of MDS 3.0, Medicare/Medicaid rules, and PCC usage.

Qualifications

  • Current and active RN license in California required.
  • Minimum 1 year of MDS Coordinator/MDS nurse experience in SNF or long-term care setting.
  • Strong knowledge of MDS 3.0, RAI process, Medicare/Medicaid reimbursement systems.

Responsibilities

  • Coordinate and oversee completion of the MDS/RAI process per regulations.
  • Collaborate with nursing, therapy, dietary, social services to ensure timely assessments.
  • Schedule and submit Medicare/Medicaid/OBRA/PPS assessments.
  • Review records to accurately capture residents' conditions and care needs.
  • Monitor MDS data for accuracy and timeliness.
  • Participate in care plan meetings and interdisciplinary teams.
  • Educate staff on MDS documentation requirements and best practices.
  • Stay updated on MDS guidelines and regulatory changes.

What We're Looking For

We are seeking an experienced and dedicated MDS-RN (Minimum Data Set Coordinator) to join our team and make a meaningful impact in the lives of our residents. The ideal candidate will be responsible for coordinating, completing, and managing the Resident Assessment Instrument (RAI) process, ensuring accurate and timely MDS assessments, care planning, and compliance with Medicare, Medicaid, and state reimbursement regulations.

This role requires strong clinical assessment skills, attention to detail, regulatory knowledge, and the ability to collaborate effectively with an interdisciplinary team.

What You'll Do
  • Coordinate and oversee the completion of the MDS/RAI process in accordance with federal and state regulations.

  • Collaborate with nursing, therapy, dietary, social services, and other departments to ensure timely and accurate assessment completion.

  • Schedule, complete, and submit all required Medicare, Medicaid, OBRA, and PPS assessments.

  • Calculate and manage Medicare assessment schedules and PPS deadlines.

  • Review medical records and clinical documentation to accurately capture residents' conditions and care needs.

  • Ensure documentation supports reimbursement requirements and regulatory compliance.

  • Monitor MDS data for accuracy, completeness, and timeliness.

  • Participate in care plan meetings and interdisciplinary team conferences.

  • Assist in developing and updating individualized resident care plans based on assessment findings.

  • Educate and support staff regarding MDS documentation requirements and best practices.

  • Maintain current knowledge of MDS guidelines, reimbursement methodologies, and regulatory changes.

  • Participate in quality assurance and performance improvement initiatives.

  • Identify documentation or compliance concerns and recommend corrective actions.

  • Foster positive relationships with residents, families, physicians, and interdisciplinary team members.

  • Adhere to all company policies, procedures, and professional standards.

What You Bring
  • Current and active Registered Nurse (RN) license in the State of California required.

  • Minimum one (1) year of MDS Coordinator/MDS Nurse experience in a skilled nursing facility (SNF) or long-term care setting required.

  • Strong knowledge of MDS 3.0, RAI process, Medicare, Medicaid, and skilled nursing reimbursement systems.

  • Experience with care planning, case management, and regulatory compliance.

  • Proficiency with PointClickCare (PCC) and electronic health records preferred.

  • Excellent organizational, analytical, and time management skills.

  • Strong written and verbal communication skills.

  • Ability to work independently while collaborating effectively with interdisciplinary teams.

  • Current CPR certification preferred.

  • RAC-CT certification is a plus.
