Job summary

Preferred Care Health Centers seeks an experienced MDS Coordinator (RN) to oversee the RAI process in Maryland. You will ensure timely MDS assessments, develop resident care plans with the interdisciplinary team, and maintain regulatory compliance.

Role requires an MD RN license, 2+ years MDS experience in long-term care, and strong knowledge of CMS guidelines and EMR systems. Collaboration with staff and leadership is essential.

Qualifications

  • Active Registered Nurse (RN) license in Maryland is required.
  • Minimum of 2 years of MDS experience in a skilled nursing or long-term care setting.
  • Strong knowledge of RAI process, CMS guidelines, and reimbursement systems.
  • Experience with electronic medical records (EMR) systems.
  • Excellent assessment, organizational, and communication skills.
  • Ability to work independently and collaboratively.

Responsibilities

  • Coordinate and complete MDS assessments per CMS guidelines.
  • Ensure timely and accurate submission of MDS assessments.
  • Develop and update individualized care plans with the interdisciplinary team.
  • Monitor documentation for regulatory compliance.
  • Participate in Medicare/Medicaid reimbursement processes.
  • Conduct care plan meetings with residents and families.
  • Educate and support nursing staff on MDS processes.
  • Perform audits and QA reviews related to MDS and care planning.
  • Stay current with regulatory changes and best practices.

Position Summary

We are seeking an experienced and detail-oriented MDS Coordinator (Registered Nurse) to join our team in Maryland. The MDS Coordinator is responsible for overseeing the Resident Assessment Instrument (RAI) process, ensuring accurate and timely completion of MDS assessments, care planning, and compliance with state and federal regulations.

Key Responsibilities
  • Coordinate and complete Minimum Data Set (MDS) assessments in accordance with CMS guidelines
  • Ensure timely and accurate submission of MDS assessments
  • Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team
  • Monitor documentation to ensure compliance with regulatory standards
  • Participate in Medicare and Medicaid reimbursement processes
  • Conduct care plan meetings with residents and families
  • Educate and support nursing staff on MDS processes and documentation requirements
  • Perform audits and quality assurance reviews related to MDS and care planning
  • Stay current with changes in regulations and best practices
