Job summary

Personal Healthcare Management LLC in New York is seeking an experienced MDS Coordinator to complete MDS assessments and care plans, initiate and support care planning, and keep documentation current. The role requires an active NY State RN license and extensive long-term care experience.

The position is full-time and in-person in the Village of Sodus with a competitive salary and benefits package. The candidate will manage care plans, collaborate with the DON and staff, and ensure

Qualifications

• Valid NY State RN License.
• Long Term Care experience required.
• Highly organized, professional & motivated.
• Working knowledge of nursing services, nursing administration, rehabilitation, geriatric nursing.
• Strong managerial, organizational, planning, computer and interpersonal abilities to initiate, complete and update care plans.

Responsibilities

• Complete accurate MDS & care plans as assigned.
• Initiate care plans and supporting activities as assigned.
• Maintain & update all care plans and assessments as required.
• Identify problems and communicate those problems to the DON and staff concerned.
• Protect the confidentiality of Resident & Facility information.

Duties Include:

• Completing accurate assessments, MDS & care plans as assigned.

• Maintaining & updating all care plans and assessments as required.
• Ability to identify problems and to communicate those problems to the DON and staff concerned
• Always Protecting the confidentiality of Resident & Facility information.

Requirements Include:

• Must be highly organized, professional & motivated
• Must have working knowledge of nursing services, nursing administration, rehabilitation, general nursing, geriatric nursing
• Must have good managerial, organizational, plannings, computer, and interpersonal ability to initiate, complete and update care plans
