Job summary

Wayne County Rehab is seeking an MDS Coordinator for our Skilled Nursing Facility. The role involves completing MDS assessments, initiating and updating care plans, and coordinating with the DON and staff to ensure high-quality resident care.

Candidate must hold a valid NY State RN license and have long-term care experience. The position is full-time, in-person, with a salary of $95,000–$100,000 and a benefits package including 401k, health/dental/vision insurance, and PTO.

Qualifications

• Valid NY State RN License.
• Long Term Care Experience Required.
• Highly organized, professional and motivated.
• Working knowledge of nursing services, nursing administration, rehabilitation, geriatric nursing.
• Ability to initiate, complete and update care plans with strong computer and interpersonal skills.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned.
• Initiating care plans and supporting activities as assigned.
• Maintaining and updating all care plans and assessments as required.
• Identify problems and communicate them to the DON and staff concerned.
