Job summary

Talentify is seeking an MDS Coordinator in Evansville, IN to complete accurate assessments, MDS and care plans, and initiate care plans as assigned.

You will create monthly care plan calendars, maintain all care plans, monitor clinical records, and keep documentation compliant while protecting resident confidentiality.

A valid LPN or RN license and prior MDS and long-term care experience are required; 40-hour workweek; strong organization and teamwork.

Responsibilities

• Monitoring & auditing clinical records, ensuring accuracy & timeliness.
• Informing DON of persistent issues related to non-compliant documentation.
• Protecting the confidentiality of Resident & Facility information at all times.
