Job summary

Fusion Healthcare Ltd in Watertown, MA seeks an experienced Registered Nurse with MDS 3.0 expertise to join our long-term care team. You will perform accurate assessments, initiate care plans, and prepare monthly care plan calendars while ensuring compliant documentation.

The role requires MDS 3.0 experience and 2 years in long-term care, plus a NYS RN license, with the ability to work 40 hours per week. We offer health, dental, vision insurance, retirement plan, and paid time off.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non-compliant documentation
• Protecting the confidentiality of Resident & Facility information at all times

Requirements:

• Valid RN License

• MUST HAVE PRIOR MDS 3.0 EXPERIENCE

• Long Term Care Experience Required!

• Must be able to work 40 hours a week

• Should have solid computer skills

• Excellent communication skills

• Should be friendly and a team worker
