Job summary

Envive Healthcare is seeking an MDS Coordinator to join our team in Kentucky. The role emphasizes accurate MDS and care plan creation, timely updates, and maintenance of comprehensive documentation.

You will coordinate with nursing staff to implement care plans, monitor records for precision, and protect resident confidentiality. The position requires prior MDS experience and an active LPN/RN license, with a 40-hour workweek in a long-term care setting.

• Informing DON of persistent issues related to non-compliant documentation.

MDS Coordinator Duties and Responsibilities:

• NO ON CALL ROTATION

• Completing accurate assessments, MDS & care plans as assigned

• Initiating care plans and supporting activities as assigned

• Creating and distributing monthly care plan calendars in a timely fashion

• Maintaining & updating all care plans and assessments as required

• Monitoring & auditing clinical records, ensuring accuracy & timeliness

• Informing DON of persistent issues related to non-compliant documentation

• Protecting the confidentiality of Resident & Facility information at all times

MDS Coordinator Qualifications and Skills:

• Valid LPN or RN License

• Must have prior MDS Experience

• Long Term Care Experience Required

• Must be able to work 40 hours a week

• Must be highly organized, professional & motivated

• Should have solid computer skills

• Excellent communication skills

• Should be friendly and a team worker

• Nurse License (Required)
