Healthcare Nursing Center is seeking an experienced MDS Coordinator to lead the RAI process in our skilled nursing and long-term care facility. You will coordinate MDS assessments, ensure accuracy and timely submission to regulatory agencies, and support compliance with federal and Pennsylvania state regulations.

The role requires RN license in PA, at least 2 years in skilled nursing, and strong knowledge of RAI, PDPM, CMS guidelines.

Qualifications

• RN license in PA required.
• Minimum of 2 years of experience in a skilled nursing or long-term care setting.

Responsibilities

• Coordinate and complete MDS assessments in accordance with federal and state guidelines.
• Ensure timely and accurate submission of MDS assessments to regulatory agencies.
• Develop, review, and update resident care plans based on assessment findings.
• Collaborate with interdisciplinary team members to ensure quality resident care.
• Monitor and maintain compliance with CMS regulations and Pennsylvania State Requirements.
• Participate in care plan meetings with residents and families as needed.
• Conduct audits to ensure documentation accuracy and regulatory compliance.
• Provide education and guidance to staff regarding MDS processes and documentation.

We are seeking an experienced and detail-oriented MDS Coordinator to join our skilled nursing and long-term care team. The MDS Coordinator is responsible for coordinating the Resident Assessment Instrument (RAI) process, ensuring accurate and timely completion of MDS assessments, and supporting compliance with federal and Pennsylvania state regulations.

Work Schedule:

Key Responsibilities:

• Coordinate and complete Minimum Data Set (MDS) assessments in accordance with federal and state guidelines
• Ensure timely and accurate submission of MDS assessments to regulatory agencies
• Develop, review, and update resident care plans based on assessment findings
• Collaborate with interdisciplinary team members to ensure quality resident care
