Qualifications

• Strong knowledge of the RAI manual, CMS guidelines, and PDPM.
• RAC-CT certification preferred but not required.
• Proficiency with EHR software (PointClickCare).
• Excellent organizational, analytical, and communication skills.

Responsibilities

• Coordinate and oversee the timely completion of the MDS assessment process (PPS and OBRA).
• Collaborate with the IDT to develop and update resident-centered care plans.
• Review clinical documentation to ensure accurate reflection of care and MDS coding.
• Monitor and manage the MDS transmission validation to the state database.
• Analyze QM data to identify clinical improvement opportunities and support QAPI initiatives.
• Stay updated on CMS guidelines, PDPM reimbursement structures, and Medicaid regulations.

Aviate in Piscataway is seeking a full-time, MDS Coordinator to join our team. In this role, you will oversee the Resident Assessment Instrument (RAI) process, ensuring accurate Minimum Data Set (MDS) assessments and care plans that drive exceptional resident care and appropriate reimbursement.

Responsibilities

• Coordinate and oversee the timely completion of the MDS assessment process (including PPS and OBRA assessments) in accordance with state and federal guidelines.

• Collaborate with the interdisciplinary team (IDT) to develop, review, and update comprehensive, resident-centered care plans.

• Review clinical documentation to ensure it accurately reflects the care provided and supports MDS coding.

• Monitor and manage the validation process of MDS transmissions to the state database, correcting any errors promptly.

• Analyze Quality Measures (QM) data to identify areas for clinical improvement and support quality assurance (QAPI) initiatives.

• Stay updated on changes to CMS guidelines, PDPM reimbursement structures, and state Medicaid regulations.
