Qualifications

• Active RN or LPN license in Illinois.

Responsibilities

• Conduct resident assessments using the MDS tool and document findings.
• Accurately record medical history, functional status, cognition and care needs.
• Create individualized care plans based on MDS data and assessments.
• Collaborate with physical, occupational, and dietary teams to coordinate care.
• Ensure timely and compliant submission of MDS assessments.
• Communicate resident needs to families and care teams, advocating for residents.
• Participate in quality improvement projects to enhance care and compliance.
