Castle Healthcare in Indiana is seeking a Registered Nurse to serve as MDS/Care Plan Coordinator. You will ensure accurate and timely completion of Medicare/Medicaid case-mix documents and PPS submissions, integrating nursing, dietary, social recreation, restorative, rehabilitation and physician services to optimize reimbursement.

The role requires current RN licensure, long-term care experience, and strong ability to read, write, speak and understand English.

Formación

• Registered Nurse with current unencumbered state licensure.
• Long Term Care Experience preferred.
• Ability to read, write, speak and understand English.

Responsabilidades

• Assesses health status and level of care of new admissions.
• Ensures timely completion of all MDS assessments (PPS Medicare, quarterly, annual, significant change).
• Communicates level of care to all disciplines.
• Coordinates interdisciplinary participation in completing the MDS within regulatory time frames.
• Maintains accurate schedule of MDS assessments with proper reference dates.
• Responsible for data entry and electronic submission of MDS assessments.
• Verifies electronic submissions and maintains records.
• Schedules and conducts resident care conferences per regulations.
• Assists in formulating and revising care plans; ensures appropriate nursing interventions.
• Evaluates care plans for comprehensiveness and individuality.
• Manages level of care changes and notifies departments.
• Generates forms for acuity changes and transmits to agencies as required.
• Other duties as assigned.
