Job summary

Qualifications

• Registered Nurse with current unencumbered state licensure.
• Long Term Care experience preferred.
• Ability to read, write, speak and understand the English language.

Responsibilities

• Assess health status and determine level of care for new admissions.
• Ensure timely completion of MDS assessments (PPS Medicare, quarterly, annual, significant change).
• Coordinate interdisciplinary participation in completing the MDS with reference dates.
• Maintain an accurate schedule of all MDS assessments.
• Enter data and submit MDS electronically; verify submissions and correct as needed.
• Schedule and conduct resident care conferences per regulations; ensure MDS reviews are complete.
