Job summary

Arcadia Care is seeking a MDS / Care Plan Coordinator to ensure timely, accurate resident assessments and care plans that meet Federal and State guidelines in a hybrid work setting in Illinois.

You will integrate nursing, dietary, social recreation, restorative, rehabilitation, and physician services, complete Medicare/Medicaid case-mix documents, and lead MDS reviews to optimize RUG categories and reimbursement.

Qualifications

• Registered Nurse or Licensed Practical Nurse with current unencumbered state licensure.
• Long Term Care Experience preferred.
• Ability to read, write, speak, and understand the English language.

Responsibilities

• Integrate nursing, dietary, social recreation, restorative, rehabilitation, and physician services to ensure appropriate assessment and reimbursement.
• Complete Medicare/Medicaid case-mix documents to assure appropriate reimbursement for care and services provided within the Facility.
• Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category.
• Oversees the overall process and tracking of MDS/PPS documentation and submission.
• No On-Call
• Remote Work Structure - One day per week
