Job summary

Arc at Sangamon valley in Springfield, IL is seeking an MDS / Care Plan Coordinator to ensure timely, accurate resident assessments and interdisciplinary care plans. You will identify resident acuity, determine care needs, and communicate needs to families and responsible parties.

Qualifications

• RN licensed with current state licensure.
• Long term care experience preferred.
• Must read, write, speak and understand English.

Responsibilities
