Job summary

Arcadia Care in Bloomington, IL is seeking a MDS / Care Plan Coordinator to ensure timely, accurate resident assessments and interdisciplinary care plans in line with Federal and State guidelines.

You will integrate nursing, dietary, social, restorative and physician services to optimize assessments and reimbursement, oversee MDS/PPS documentation, and conduct regular MDS reviews. Remote work structure allows one day per week.

Qualifications

• Registered Nurse or Licensed Practical Nurse with current unencumbered state licensure.
• Long Term Care experience preferred.
• Ability to read, write, speak and understand English.

Responsibilities

• Integrate nursing, dietary, social recreation, restorative, rehabilitation, and physician services to ensure appropriate assessment and reimbursement.
• Complete Medicare/Medicaid case-mix documents to assure appropriate reimbursement for care and services provided within the Facility.
• Conduct continual Minimum Data Set (MDS) reviews to assure achievement of optimal RUG category.
• Oversee the process and tracking of MDS/Prospective Payment System (PPS) documentation and submission.
• No On-Call.
• Remote Work Structure - One day per week.
