Job summary

Arcadia Care is seeking an MDS / Care Plan Coordinator to ensure timely completion of accurate resident assessments and interdisciplinary care plans that meet Federal and State guidelines.

You will integrate nursing, dietary, social recreation, restorative, rehabilitation, and physician services to support appropriate assessment and reimbursement, and you will oversee MDS/PPS documentation and submission with a focus on optimal RUG categorization.

Qualifications

• Registered Nurse or Licensed Practical Nurse with current unencumbered state licensure.
• Long Term Care experience preferred.
• Ability to read, write, speak, and understand English.

Responsibilities

• Ensure timely completion of accurate resident assessments and care plans that meet Federal and State guidelines.
• Integrate nursing, dietary, social recreation, restorative, rehabilitation, and physician services for appropriate assessment and reimbursement.
• Complete Medicare/Medicaid case-mix documents to support reimbursement.
• Conduct ongoing MDS reviews to optimize RUG category.
• Oversee MDS/PPS documentation and submission processes.
• Support a remote work structure with one day per week remote work.
