Job summary

Arcadia Care in Kewanee, IL seeks a MDS / Care Plan Coordinator to ensure timely, accurate resident assessments and interdisciplinary care plans meeting Federal and State guidelines. You will identify acuity, determine care needs, and convey expectations to families and responsible parties.

Responsibilities include integrating nursing, dietary, social, and physician services, managing Medicare/Medicaid case-mix documents, and leading MDS/PPS documentation with no on-call duties.

Qualifications

• Active state licensure as RN or LPN.
• Long Term Care experience preferred.
• English language proficiency required.

Responsibilities

• Coordinate MDS and care plans ensuring Federal/State compliance.
• Identify resident acuity and care needs; communicate with families.
• Oversee MDS/PPS documentation and reimbursement processes.
• Collaborate with interdisciplinary team to integrate services.
• No On-Call; Remote Work Structure - One day per week.
