Job summary

Arcadia Care is seeking a MDS / Care Plan Coordinator to ensure timely, accurate resident assessments and interdisciplinary care plans that meet federal and state guidelines. The role involves identifying resident acuity, determining care needs, and communicating expectations to families and responsible parties.

Essential duties include integrating multiple disciplines for proper assessment, completing Medicare/Medicaid case-mix documentation, and overseeing MDS/PPS submissions to support

Qualifications

• Registered Nurse with current unencumbered state licensure.
• Long Term Care experience preferred.
• Ability to read, write, speak, and understand English.

Responsibilities

• Integrate nursing, dietary, social recreation, restorative, rehab, and physician services for assessments and reimbursement.
• Complete Medicare/Medicaid case-mix documents to assure appropriate reimbursement.
• Conduct continual MDS reviews to optimize RUG category.
• Oversee MDS/PPS documentation and submission.
