Job summary

Nadia Care is seeking a full-time Maternity Navigator to support expectant and new mothers in East Baltimore and the Harford County border, Maryland. You will work with a multidisciplinary team to connect families with care, education, and community resources in person, virtually, or at home.

The role centers on empathetic, culturally sensitive coaching on prenatal and postpartum health, documenting interactions, and coordinating with specialists to ensure safe, joyful pregnancies.

Qualifications

• 3+ years in community health, patient advocacy, or care navigation.
• Experience providing prenatal and postpartum health education.
• Excellent listening, communication, and cultural sensitivity.
• Ability to manage multiple priorities in a fast-paced environment.
• Independent field work with team support.
• Valid driver's license and reliable transportation.
• Access to home internet and a phone with data.
• Proficient with documentation in AthenaOne, Welkin, and MacOS.

Responsibilities

• Meet with mothers virtually and in person, educating on urgent maternal signs, preeclampsia, and postpartum recovery.
• Educate on birth positions and breathing techniques to build confidence.
• Collaborate with a multidisciplinary team and elevate clinical issues to specialists.
• Maintain clear documentation of care interactions to ensure coordinated support.
• Build trusting relationships and provide support via calls or messaging between visits.
• Connect families to community resources and overcome barriers like housing or transportation.
• Represent community needs to shape Nadia Care programs.
• Attend administrative meetings and follow policies to ensure high standards.
