The role emphasizes field work with hybrid scheduling, compassionate care, and collaboration with a multidisciplinary team to uplift clinical outcomes through education and advocacy.

Qualifications

• 3+ years in community health, patient advocacy, or healthcare navigation as a certified professional.
• Experience providing prenatal and postpartum health education.
• Strong listener and communicator with cultural sensitivity.

Responsibilities

• Meet mothers virtually and in-person, educating on urgent maternal signs, preeclampsia, and postpartum recovery.
• Educate on birth positions and breathing techniques pre-birth.
• Collaborate with multidisciplinary care teams and attend daily huddles.
