Job summary

Nadia Care is seeking a full-time Maternity Navigator to serve expectant and new mothers in East Baltimore and the Harford County border, Maryland. You will build deep, trust-based relationships, educate families on key health topics, and connect them to community resources to remove barriers to a healthy pregnancy.

You’ll work in a hybrid schedule, with both in-person and remote support, and earn $18–$28 per hour depending on experience.

Qualifications

• 3+ years in community health, patient advocacy, or healthcare navigation
• Certified Perinatal CHW, CMA, or CNA preferred
• Prenatal and postpartum health education experience
• Comfort delivering education both in-person and virtually

Responsibilities

• Meet mothers virtually and in-person, educating on urgent maternal warning signs and postpartum recovery
• Educate on birth positions and breathing techniques
• Work with a multidisciplinary care team and attend daily huddles
• Maintain clear documentation of care interactions
• Build trusting relationships and provide support via calls or messaging between visits
• Connect families to community resources to overcome barriers like housing or transportation
• Provide community feedback to shape Nadia Care programs
• Attend administrative meetings and follow policies and procedures
