Job summary

Nadia Care is seeking a full-time Maternity Navigator to support expectant and new mothers in the East Baltimore and Harford County border, Maryland area. You will bridge gaps between the healthcare system and the community, educating mothers and coordinating care to ensure healthy, joyful pregnancies.

Qualifications include 3+ years in community health/navigation and credentials such as a Certified Perinatal Community Health Worker or CMA/CNA.

Qualifications

• 3+ years of experience in community health, patient advocacy, or healthcare navigation as a Certified Perinatal Community Health Worker or CMA/CNA.
• Deep understanding of challenges expectant mothers face and experience providing prenatal/postpartum health education.
• Excellent listening and communication with compassion and cultural sensitivity.
• Strong problem solving and ability to manage multiple priorities in a fast-paced environment.
• Comfort working independently in the field while staying connected to a team of specialists.
• Valid driver’s license with clean driving record and reliable transportation.
• Access to home internet and a phone with internet access.
• Tech-savvy and able to learn AthenaOne, Welkin and MacOS.

Responsibilities

• Meet with mothers virtually and in-person, educating them on critical health topics.
• Educate on birth positions and breathing techniques.
• Work with a multidisciplinary care team and attend daily huddles.
• Maintain clear and accurate documentation of all care interactions.
• Build trusting relationships and provide support via call or messaging between visits.
• Connect mothers to community resources to overcome barriers like housing or transportation.
• Provide insights to shape Nadia Care programs.
• Attend administrative meetings and follow policies.
