Legacy Community Health is seeking a Bilingual Perinatal Navigator to support pregnant individuals and families through pregnancy, postpartum, and early childhood. You will help patients access care, education, and community resources, coordinating with an integrated clinic and care team.

This entry-level role may be grant-funded and requires compassion, organization, and a commitment to patient-centered care.

Qualifikationen

• Excellent communication skills and customer service, both oral and written.
• Basic understanding of statistics and graphical representation principles.
• Ability to manage multiple deadlines and prioritize tasks.
• Demonstrated competency with computers and applications, including Microsoft Office Suite.
• This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management.
• Bilingual English/Spanish fluency.

Aufgaben

• Conduct point-of-care testing, including HIV testing, pregnancy testing, and related services.
• Conduct health screenings and risk assessments.
• Assist clients with scheduling patient appointments and follow-ups.
• Conduct outreach to identify and engage pregnant individuals in need of support.
• Provide culturally sensitive education on prenatal care, nutrition, and healthy pregnancy practices.
• Document all patient services in the electronic health record in the Legacy EMRS.
• Promote effective working relationships with internal and external stakeholders.
