The University of Texas at Austin is seeking a Community Health Worker to serve as a trusted liaison between the healthcare system and postpartum families. You will navigate medical and social services, provide culturally responsive health education, and connect participants to housing, food, and transportation resources.

Qualifications

• High School Diploma or GED required.
• At least 1 year of experience in community outreach, health education, or social services.
• DSHS CHW credential or obtain within 6 months of hire.

Responsabilités

• Conducts Outreach and Engagement: builds relationships with participants and families; educates on health and social services; screens for social needs; maintains trust with postpartum mothers; provides culturally responsive support.
• Navigates Health and Social Systems: helps access medical, behavioral, and social services; assists with applications for insurance, housing, and food; tracks referrals and ensures completion.
• Provides Health Education and Coaching: delivers tailored health education; supports behavior change; reinforces care instructions; assists with medication adherence and appointment follow-ups.
• Documents and Reports Participant Interactions: maintains HIPAA-compliant records in RedCap; reports data for quality metrics; communicates progress to care team.
• Supports Community-Based Initiatives: collaborates with partners; advocates for resources; contributes to program evaluation and health equity.
