Job summary

Legacy Baytown is seeking a Bilingual Perinatal Navigator to support pregnant individuals and families, guiding them to care, education, and community resources from pregnancy through postpartum. This entry-level role emphasizes building trust, reducing barriers, and promoting healthy outcomes within an integrated clinic setting.

You will work with the care team to connect patients to OB/pediatric services, eligibility services, and appointment coordination, with potential grant-funded aspects.

Qualifications

• Two (2) years of relevant experience is required.
• Bilingual English/Spanish fluency is required.
• Strong communication and organizational skills are essential.

Responsibilities

• Conduct point-of-care testing (HIV, pregnancy) and related services.
• Conduct health screenings and risk assessments.
• Assist clients with scheduling patient appointments and follow-ups.
• Conduct outreach to identify and engage pregnant individuals in need of support.
• Provide culturally sensitive education on prenatal care, nutrition, and healthy pregnancy practices.
• Document all patient services in the electronic health record.
