Job summary

The Tulsa Health Department seeks dedicated individuals for Healthy Start case management in Tulsa County. You will provide comprehensive case management to pregnant women and families, including intake, screening, and referrals, while coordinating with medical and social services to improve birth outcomes.

Minimum bachelor's degree in social work or related field is required; three years of case management experience is preferred. A valid Oklahoma driver’s license is required where applicable.

Qualifications

• Bachelor's degree in social work or related field required.
• Three years of case management experience; MCH preferred.
• Bilingual - Spanish may be required.
• Eligible for Oklahoma social worker licensure; valid Oklahoma driver's license.

Responsibilities

• Provide case management including screening, assessment, and education.
• Complete intake, screenings, and risk assessments for clients.
• Make referrals to community resources and follow up on appointments.
• Coordinate with medical, behavioral health, and social services.
• Maintain HIPAA confidentiality and proper documentation.
• Travel to clients' homes and work with families toward goals.
