The role requires a Bachelor’s in social work or related field, 3 years’ experience, licensure eligibility, bilingual skills, and a valid Oklahoma driver’s license. Exempt, full-time with comprehensive benefits.

Qualifications

• Bachelor’s degree in social work or related field is required.
• Master’s degree in health or related field preferred.

Responsabilités

• Provide case management services including screening, assessment, and education.
• Determine intervention strategy per Healthy Start protocols.
• Complete client documentation and ensure HIPAA compliance.
• Recruit and maintain appropriate caseload levels.
• Interview clients and perform intake, screening, and risk assessments.
• Coordinate with medical, behavioral health, and social services to improve care.
• Attend trainings, meetings, and contribute to quality assurance activities.
