Qualifications

• High school diploma or GED with 3+ years in social service/health field.
• Bachelor's degree preferred; combinations considered.
• Experience with home visiting and health disparities favored.
• Ability to work in field ~95% of time.

Responsibilities

• Identify and enroll eligible participants.
• Provide home- and community-based case management and coordination.
• Maintain a caseload of up to 48 participants.
• Develop and manage plans of care including birth and reproductive life plans.
• Provide education on breastfeeding, immunization, safe sleep, nutrition, etc.
• Link participants to housing, transportation, food, and mental health services.
