Job summary

Family & Children's Services seeks an on-site case manager to provide holistic support to pregnant individuals and families. You will coordinate resources across health, housing, education, and employment, acting as a single point of contact in a multidisciplinary team to help clients access necessary services for recovery.

The role involves ODMHSAS/Medicaid billing, advocacy, crisis intervention, and linkage to community resources to ensure comprehensive, culturally sensitive care.

Qualifications

  • Bachelor’s Degree in psychology or related field required.
  • Case Management Certification required or to be obtained after hire.
  • Must have a valid Driver license and access to a vehicle.

Responsibilities

  • Provide on-site case management services to pregnant individuals and families.
  • Link families to health, housing, education, and employment resources.
  • Coordinate referrals and advocate for clients with ODMHSAS and Medicaid billing.
  • Work within a multidisciplinary team to support client recovery.
  • Perform crisis intervention, education, and monitoring as needed.

Provide on-site case management services, support groups, and socialization opportunities to individuals who are pregnant or who have recently been pregnant, and their families. Link families with basic needs, physical health appointments, mental health appointments, housing issues, and education and employment opportunities. Including arranging, coordinating, planning, advocacy, linkage, referral, education, crisis intervention, resource acquisition, and monitoring in a culturally sensitive way. Provide a single point of contact and work in partnership with others as part of a multidisciplinary team to provide a set of social service functions that helps clients access the resources they need to recover.


Responsible for Oklahoma Department of Mental Health and Substance Abuse Services and Medicaid billing.


Requirements:



  • Bachelor’s Degree in the social sciences field is required, with consideration given for degrees in Criminal Justice or Education.

  • Case Management Certification or obtain by taking the first available Case Management Certification course provided by ODMHSAS after hire.

  • Must have a strong commitment to the right and ability of each person served to live in normal community residences; and have access to helpful, adequate, competent, and continuous supports and services.

  • Skills and competence to establish supportive trusting relationships with persons with mental illnesses and/or substance abuse issues and respect for client rights and personal preferences in treatment are essential.

  • Must possess a valid Driver License and satisfactory driving record and use personal automobile to travel to locations other than primary office.


CCBHC Model of Care


Family & Children's Services works to heal hurting and abused children, strengthen families, and provide hope and a path to recovery for those battling mental illness and addiction. You will be working in an innovative and dynamic environment using a new transformative model of care Certified Community Behavioral Health Clinic (CCBHC). This model is characterized by innovative, team-based approach and whole person care for adults and children with a range of complex mental health and substance use challenges.


Utilizing multi-disciplinary teams, staff work collaboratively to provide a coordinated effort to enhance client recovery. Robust CCBHC services often result in better client outcomes and quality of care due to:



  • Increased access to care and crisis services

  • Expanded traditional community mental health and substance use services.

  • Added Care Coordination and physical health screening for mental health clients

  • Greater access to Social Services for clients’ economic and social needs

  • Increased specialized services for veterans, those most in need, and those impacted by the opioid crisis.
