Job summary

Hamakua-Kohala Health seeks a Maternal Care Coordinator to manage prenatal caseloads, coordinate services, educate on infant care and postpartum health, and facilitate follow-up through one year postpartum.

The role requires strong customer service, organization, and the ability to work with physicians, nurses, and families in a medical office environment. Hawaii driver’s license is required.

Qualifications

• High school graduate or equivalent. Must possess extraordinary customer service skills.
• Excellent organizational skills and ability to multi-task.
• Must be able to use a computer, type and comprehend computer logic.

Responsibilities

• Care coordination to ensure high quality care services.
• Assist patients as a navigator through the healthcare system.
• Collaborate with providers and staff to promote timely, preventive care and connect to resources.
• Manage databases of insurance carriers to identify care gaps and promote patient compliance.
• Participate in data collection and quality improvement initiatives.
• Travel to and from clinic locations as needed.
