The Tulsa Health Department is seeking a Family Navigator to support the Birth through Eight Strategy for Tulsa (BEST). You will connect families with home visitation and early childhood programs and help pregnant women enrolled in SoonerCare access prenatal and postpartum supports.

This full-time role emphasizes outreach, case management, collaboration with partners, and accurate documentation to improve outcomes for children in Tulsa County.

Qualifications

• Education: High school diploma required. Associates Degree preferred.
• Experience with low-income families, home visitation programs and early childhood services preferred.
• Bilingual abilities in English/Spanish/Burmese preferred.

Responsabilités

• Create welcoming, inclusive environment with expectant mothers, parents and caregivers.
• Triage referrals and connect to home visitation and other programs.
• Educate families about prenatal and postpartum supports and brain development.
• Refer families to medical, education and social services as needed.
• Maintain accurate documentation of activities and referrals.
