Qualifications

• HS diploma or GED; preferred Bachelor’s or Master’s in social/health services.

Responsibilities

• Provide high quality, compassionate guidance in individual and group counseling.
• Plan, oversee, facilitate and document patient recovery.
• Co-facilitate group or family sessions as needed.
• Ensure all documentation of care, treatment, and incidents is timely and clear.
• Prepare individual treatment plans for assigned patients.
• Conduct initial and follow-up assessments.
• Evaluate patient needs and refer as needed to other programs.
• Assist with aftercare planning and crisis intervention when required.
• Provide case management to ensure individualized care and liaison with referrals.
