Job summary

Wexford Health Sources, Inc. in Memphis, TN, seeks an MOUD/MAT Nurse (LPN) to administer, monitor, and coordinate MOUD medications for patients in the Shelby County Divisions of Corrections.

You will work on day shift with a focus on induction, stabilization, and maintenance phases and collaborate with the treatment team to ensure continuity of care. The ideal candidate holds a current LPN license, CPR certification, and at least two years of nursing experience.

Qualifications

• Current, unrestricted LPN license in state of practice.
• Current CPR certification.
• Minimum of two years of nursing experience preferred.

Responsibilities

• Administer and monitor MOUD medications, including observed dosing, injections, and oral formulations.
• Assess and monitor patients through induction, stabilization, and maintenance phases of treatment.
• Collaborate with providers, behavioral health staff, pharmacies, and treatment team members to coordinate patient care.
• Respond to sick calls, provide patient education, conduct medication monitoring, and support continuity of care.
• Advocate for patients while promoting a non-judgmental, recovery-focused approach to treatment.
• Maintain accurate documentation and ensure compliance with clinical, regulatory, and confidentiality requirements.

Position Summary

What You’ll Need
