Job summary

Wexford Health Sources, Inc. seeks a MOUD/MAT Nurse (LPN) for Guadalupe Correctional Facility in Santa Rosa, NM. Day shift, 40 hrs/wk, you will administer and monitor MOUD, perform patient assessments, and coordinate care with an interdisciplinary team.

Qualifications

• Currently licensed Practical Nurse (LPN) in state of practice and CPR certification.
• Minimum of two years of nursing experience preferred.

Responsibilities

• Administer and monitor MOUD medications, including observed dosing, injections, and oral formulations
• Assess and monitor patients throughout induction, stabilization, and maintenance phases of treatment
• Collaborate with providers, behavioral health staff, pharmacies, and treatment team members to coordinate patient care
• Respond to sick calls, provide patient education, conduct medication monitoring, and support continuity of care
• Serve as a patient advocate while promoting a non-judgmental, recovery-focused approach to treatment
• Maintain accurate documentation and ensure compliance with clinical, regulatory, and confidentiality requirements

Position Summary

As a MOUD/MAT Nurse, you'll provide skilled nursing care and medication-assisted treatment services to patients with opioid use disorders. Working closely with an interdisciplinary treatment team, you will:
