Job summary

Boulder County Jail is seeking a Full Time Medication Assisted Treatment (MAT) Coordinator to lead the Jail MAT (JMAT) program. You will conduct substance use assessments, provide counseling, educate inmates on MAT, and coordinate referrals to internal and community services, ensuring continuity of care from intake to post-release.

The role requires collaboration with medical, behavioral health, and custody staff, with responsibilities for documentation, outcomes tracking, and quality

Qualifications

• Knowledge of opioid use disorder, MAT, relapse prevention, and recovery-oriented care.
• Ability to conduct comprehensive substance use assessments and develop treatment plans.
• Experience coordinating Medicaid enrollment and care continuity for program participants.

Responsibilities

• Collaborate with nursing, medical, and behavioral health staff to identify inmates with OUD.
• Review referrals and meet inmates eligible for Jail MAT.
• Provide substance use assessments and education on MAT and recovery.
• Document encounters, treatment plans, and outcomes in the EHR.
• Coordinate referrals to internal jail programs and community providers.
• Ensure Medicaid eligibility/enrollment and continuity of care after release.
• Prepare reports and participate in quality improvement activities.
