Job summary

Hampden County Sheriff’s Office is seeking a Care Coordinator for the Medication Assisted Treatment (MAT) Program, primarily at the Main Institution in Ludlow and other facilities as needed. The role focuses on supporting justice-involved individuals on their recovery journey by facilitating educational and treatment groups and assisting with aftercare planning.

Responsibilities

• Facilitates psycho-educational and treatment groups; observes, engages, and builds therapeutic rapport with participants; responds to changes in behavior including de-escalation techniques.
• Maintains accurate and timely documentation; participates in treatment and staff meetings; shares program and community information; collaborates with Sheriff’s Office staff to develop behavior management plans and supportive treatment environment.
• Provides consultation and education to staff to enhance understanding and continuity of care for program participants.

In this vital role, the Care Coordinator supports justice-involved individuals on their path to recovery by facilitating educational and treatment groups and assisting with aftercare planning.

Through empathy, professionalism, and a commitment to excellence, the Care Coordinator helps foster a safe, structured, and respectful treatment environment while supporting the effective operation of a program that addresses substance use disorder and promotes long-term success.

Work schedule

Monday-Friday, 9:00 AM to 5:00 PM; must be able to work any shift as directed

What You’ll Do

The Care Coordinator facilitates psycho-educational and treatment groups; observes, engages, and builds therapeutic rapport with participants; and responds appropriately to changes in behavior, including using de-escalation techniques when necessary. This role includes maintaining accurate and timely documentation, participating in treatment and staff meetings, sharing relevant program and community information, and collaborating with Sheriff’s Office staff to develop behavior management plans and support a positive treatment environment. The Care Coordinator also provides consultation and education to staff to enhance understanding and continuity of care for program participants.
