Job summary

RiverValley Behavioral Health in Owensboro, KY seeks a MAT Case Manager to coordinate medication-assisted treatment for patients with substance use disorders. The role serves as liaison among patients, prescribers, nursing staff, and community resources to ensure continuity of care and promote recovery.

Essential duties include intake assessments, treatment planning, progress monitoring, and collaboration with a multidisciplinary team.

Qualifications

• Requires a background in social work, counseling, psychology, or a related behavioral health field.
• Bachelor's degree in a Human Services field or equivalent healthcare experience preferred.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience with children, families, and adults in crisis is preferred.

Responsibilities

• Conduct intake assessments and develop individualized treatment plans.
• Monitor patient progress and adherence to prescribed medications.
• Provide counseling, education, and referrals to supportive services.
• Coordinate care with physicians, nurses, therapists, and outside providers.
• Maintain compliant documentation and protect patient confidentiality.
• Advocate for patients' needs with harm-reduction and recovery-focused approaches.
