Job summary

Terros Health is seeking a MAT Case Manager at our McDowell Rd Health Center in Phoenix, AZ. You will work with the clinical team to engage clients in medication-assisted treatment, coordinate care, and ensure continuous connections to resources.

The role requires collaboration with family, behavioral and medical providers, with a focus on thorough documentation, follow-up, and quality program outcomes.

Qualifications

• High school diploma or GED; BHT requirements as applicable.
• Bachelor's or master's degree in behavioral health preferred.
• Experience with opiate use disorder/substance abuse.
• Proficient with MS Office; EHR experience (NextGen preferred).

Responsibilities

• Coordinate care with the clinical team for MAT clients.
• Outreach and follow-up to ensure resources; crisis intervention as needed.
• Document in the EHR the same day care is delivered; maintain communication with involved parties.
• Engage clients, educate, and coordinate care with families and other providers.
